Summary
of
responsibilities
Reporting
to
the
Manager,
Outpatient
and
Geriatric
Mental
Health
Services,
the
Mental
Health
Counsellor
–
System
Navigator
provides
support
to
individuals
with
addiction
and/or
mental
health
issues.
Working
closely
with
other
CCH
Community
Addiction
and
Mental
Health
Services
(e.g.,
Transitional
Day
Program)
and
external
partners
(e.g.,
Situation
Table,
Youth
Hub)
the
Mental
Health
Counsellor
–
System
Navigator
identifies
a
person’s
supportive
care
needs,
and
those
of
their
family
and
works
with
them
to
develop
a
community
care
plan
to
address
issues
that
led
to
hospitalization
or
emergency
department
visit.
The
Mental
Health
Counsellor
–
System
Navigator
provides
a
continuum
of
functions,
including
building
therapeutic
relationships,
client
and
systems’
advocacy,
symptom
management,
need
assessment,
discharge
planning,
life
skills
teaching,
counselling,
family
support,
crisis
intervention
and
case
coordination.
The
individual
assists
with
program
development,
implementation
and
evaluation
processes.
Requirements
of
the
job
Required:
–
2-year
Diploma
in
Social
Services
or
Health
Sciences
–
Minimum
2
year
demonstrated
knowledge
of
and
recent
clinical
experience
working
with
mental
health
and
social
services.
–
Significant
experience
in
transitional
care,
case
management
and
psycho-social
rehabilitation.
–
Excellent
analytical
and
reasoning
skills
–
Excellent
people,
time
management
and
organization
skills
–
Demonstrated
regular,
punctual
attendance
–
Demonstrated
commitment
to
patient
safety
–
Functional
level
in
both
official
languages
(oral
and
written
in
English;
oral
in
French)
–
and
testing
completed
Asset:
–
Up-to-date
knowledge
of
relevant
legislation,
including
Mental
Health
Act,
