Emergency preparedness is a critical aspect of CCH operations. It is essential that all staff and physicians receive training and understand their roles and responsibilities in this field.
Summary of responsibilities
The Quality, Patient Safety, Patient Experience and Risk Management Specialist is responsible for the knowledge creation and distillation, diffusion and dissemination, and organizational approach to patient safety and quality across CCH. The Specialist is committed to ensuring a standardized, focused, and relentless approach to safety and quality across the organization.
This will be accomplished through:
•Partnering with patients, staff, and affiliates to mitigate patient safety risks, and respond to patient safety events.
•Building a strong culture of continuous improvement and innovation across CCH and support the Continuous Improvement of Care initiatives across CCH.
•Collaborate with key stakeholders to ensure compliance with Accreditation Canada Standards and assist in the coordination of Accreditation survey activities.

General Accountabilities:
· Ensures program readiness and communication regarding the required elements of CCH’s Quality Improvement Plan (QIP)
· Highlights the results and accomplishments of the QIP and creates a platform for areas to collaborate on and share the successes of quality improvement initiatives
· Supports and collaborates with program stakeholders to assist in identifying, and leading, the accomplishment of quality improvement initiatives;
· Supports and collaborates with program stakeholders to assist in identifying quality indicators, identifying data driven opportunities and monitoring improvements to ensure the desired results have been achieved;
· Implements appropriate tools to monitor and evaluate initiative outcomes. Facilitates consultation and data collection (such as surveys, interviews, focus groups, chart audits, etc.) as needed to support the quality improvement program
• Provides consultation and root cause analysis as part of quality of care reviews, Critical Incident Reviews, and Coroner’s Inquiry Reviews, including those under the Quality of Care Information Protection Act (QCIPA), and event investigation/review including interviews with appropriate stakeholders as required;
• Supports and/or co-leads the development, implementation and dissemination of recommendations from patient safety incident reviews;
• Informs development and supports implementation of CCH Patient Safety Plan, which serves as a roadmap for safe patient care;
• Supports patient and family’s engagement in patient safety improvement activities;
• Partners with clinical and support areas to conduct Failure Mode and Effects Analysis (FMEA) to proactively identify risk to patients;
• Provides support, direction and leadership to program areas related to obligation to meet or maintain all Accreditation Canada standards;
• Reviews policies and procedures to ensure alignment with Accreditation Standards, follows up with policy owner if policies require amendments;
• Works with assigned program area to prepare for onsite surveys and helps implement process for continuous Accreditation cycle;
• Accountable for visibly role modeling the values and attributes of staff working in a hospital focused on an outstanding patient and family experience. This includes a focus on quality, safety, risk mitigation and building internal capacity with staff, physicians and volunteers to improve the patient and family experience
• In coordination with manager of the department, meets with family members and staff to understand their concerns where there has not been resolution at the departmental level
• Supports the administration of and maintenance of the Risk Management Risk Register (HIROC) and CCH Risk Framework and coordinates required activities throughout the 3-year cycle
• Provides training to relevant groups (Managers, Leadership, and Board of Directors) as required in order to maintain compliance to Accreditation Canada standards and requirements
• Lead and/or participate in training and education initiatives pertaining to the functions of the role.
Requirements of the job
• Bachelor of Science in Nursing
• Current and unrestricted registration with College of Nurses of Ontario
• Canadian Patient Safety Program Certificate (or willingness to obtain within one year of employment).
• Minimum of 3 years relevant experience
• Knowledge of governmental and other regulatory standards, requirements, guidelines and industry best practices related to; Patient Safety, Quality improvement and Accreditation
• Knowledge and experience with root cause analysis, cause and effect analysis, and failure mode and effects analyst (FMEA) as applied in healthcare
• Advanced verbal and written communication skills
• Sound judgment and initiative for priority setting and strong project management skills
• Demonstrated ability to establish effective interpersonal relationships with colleagues, clinicians, and other internal and external stakeholders
• Advanced knowledge of computers and software (e.g Microsoft Office 365/Teams)
