Summary of responsibilities

Reporting to the Manager of the Chronic Disease Management (CDM) program, the RN will be responsible for the assessment and management (planning and providing) of Chronic Disease Management patients in an outpatient clinic setting. The RN will work as part of the team providing assessment, care and teaching to the chronic disease management patient, family and caregivers.
As a member of the interdisciplinary team, the role of Registered Nurse (RN) at CCH is to deliver safe, effective, patient-centered, ethical, and evidenced-based nursing care. The overall function of the RN is to assist our patients and community to manage clinical conditions and transitions and maximize their self-care abilities as well as embody a culture of caregiver inclusion in the patient’s plan of care. The RN also acts as a resource for members of the care team. The RN will be responsible for supporting team members across the chronic disease management program to facilitate exceptional patient care.

Requirements of the job

• Holds current Certificate of Competence from the College of Nurses of Ontario
• BScN
• Holds current certification in Basic Cardiac Life Support
• Demonstrated participation in continuing education programs, seminars or other related activities to maintain chronic disease management nursing knowledge.
• Knowledge of Quality Improvement
• Demonstrated regular, punctual attendance
• Ability to meet the physical demands of the position
• Functional level in both official languages (oral and written in English; oral in French – and testing completed)
• Relevant experience in an outpatient/ambulatory clinic setting

• Excellent patient assessment, critical thinking, communication (oral and written), and problem-solving skills
• Ability to develop, implement, evaluate and adapt a therapeutic nursing plan (Care plan)
• Ability to teach and mentor nurses, students as required
• Completes established curricula as required at the unit level
• Commitment to personal continuing education activities, including self- evaluation
• Demonstrates ability and application of knowledge related to Central Venous Access Devices (CVAD), including care and maintenance
• Demonstrates leadership decision-making skills and abilities
• Knowledge of Quality Improvement
• Demonstrated commitment to patient safety
• Working knowledge of Cerner electronic health record (or alternative electronic health record)
• Good computer skills and working knowledge of Microsoft Office, email, internet, etc.

• Certified Diabetes Educator
• Telehealth experience
• Respiratory Care Certificate
• Clinical Nurse Specialist – stroke
• Special training in teaching and counselling skills
• Membership to RNAO
