Registered Practical Nurse (RPN)- BSU

The Behavioral Support Unit (BSU) is an integral part of the continuum of care to enhance the quality of life for the residents who are living with responsive behaviors and to provide support families with compassion and understanding during their stay on the BSU.
This position is for our Rouge Valley Home, located in Toronto, Ontario.

What you’ll be doing
- Ensure resident care is provided with established nursing standards and principles, physicians’ orders and administrative policies, as well as observe, report and record vital signs, symptoms and conditions of residents, and initiate remedial health measures where appropriate. Assure observance of required techniques in nursing care.
- Assess residents and develop, implement, review and evaluate resident care plans. Monitor residents’ health, note changes in condition and assess need for referral to doctors; process and implement physician orders.
- Organize and direct functions and activities related to provision of care to residents including restorative care and palliative care as required. Interprets and implements established policies, procedures, and regulations.
- Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in-service education. Monitor performance and conduct performance appraisal and refer problems to the Director of Care.
- Understands and commitments to the BSU Model of Care and the principles of Behavioural Supports Ontario (BSO)
- Protects resident’s rights and advocacy role for the resident and families
- Provides resident focused care supported by evidence-based clinical practices
- Implements best practices for dementia, delirium, depression and other mental health conditions based on scope of practice
- Contributes to the assessment, evaluation and documentation of dementia, delirium, mental and physical health by applying the techniques of the Model of Care
- Assess risks and escalate to the appropriate individuals and documents according to professional standards
- Demonstrates trouble-shooting and problem-solving skills relative to the situation within the scope of practice
- Utilizes critical thinking skills for the early detection, intervention, and prevention of responsive behaviours
- Assess and implement non-pharmacological and therapeutic behavioural approaches
- Understands pharmacological interventions, and their interactions, that are used to manage conditions that have behavioural components along the disease continuum
- Collaborates and effectively communicates in interacting with residents, family members and other members of inter-professional teams, including community and primary care team members
- To strategize, foster and enhance collaboration and communication amongst care partners
- Demonstrates leadership, facilitation, coaching & mentoring skills
- Other duties assigned
What you bring
- Extensive knowledge and skills working with individuals with various types of dementia, delirium, mental health or other neurological conditions and those who experience an acute behavioural change
- Experience in long-term care preferred
- Trained in P.E.I.C.E.S. Gentle Persuasive Approach, DementiAbility, etc. is preferred
- Demonstrates enthusiasm, flexibility and ability to cope with change
- Experience /familiarity with RAI documentation including assessments, Resident Assessment Protocols (RAPS) and care planning functions
- Good understanding of Pharmacology-drugs used to manage those living with responsive behaviours
- Good understanding of non-Pharmacological & therapeutic supportive measures approaches
- Appropriate current certificate of competence from the College of Nurses of Ontario required
- Registration or eligible for registration with the Registered Practical Nurses Association of Ontario
- Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices
- Effective human relation skills and demonstrated commitment to working with seniors.
- Effective communication skills and the ability to work independently and collaboratively are essential.
