- Provides expert nursing care and co-ordinate complex care challenges across a variety of settings through the roles of expert clinician, educator, researcher, consultant and change agent to promote quality outcomes for the geriatric patient population in alignment with hospital priorities
- Works collaboratively with the interprofessional team to achieve excellence in patient/client centred care
- Participates in the development, monitoring and evaluation of protocols/guidelines/standards of care/programs for the geriatric patient population by offering rationale/support based on (scientific) research and clinical expertise
- Participates in research designed to evaluate patient care outcomes and improve quality for care
- Participates and provides leadership on interdisciplinary committees related to clinical practice, education and research
- Demonstrate QCH values: accountability, innovation, respect
- Masters of Science in Nursing
- Registration with the College of Nurses of Ontario
- Minimum of 5 years clinical nursing experience in geriatrics
- Demonstrated ability to conduct physical examinations and cognitive status evaluations of adults
- Demonstrated ability to lead/facilitate change and introduce clinical best practice
- Demonstrated excellent communication (written and oral) skills
- Demonstrated computer skills: Microsoft Outlook, Word, Excel, PowerPoint, electronic patient documentation system
- Demonstrated ability to respond to shifting priorities and/or multiple competing priorities
- Demonstrated research, teaching and consulting skills
- Excellent organizational, interpersonal, problem solving and decision-making skills
- Proven facilitation and collaboration skills
- Knowledge of Nursing Practice Standards and Code of Ethics of the College of Nurses of Ontario and Canadian Nurses Association.
- Member of Registered Nurses Association of Ontario
- Current BCLS training
- Demonstrated excellent attendance record
- Canadian Nurses Association Certificate in Gerontological Nursing
- Member of Canadian Gerontological Nursing Association
- Member of Ontario Clinical Nurse Specialist Interest Group
