         SERVICE PROFILE

The Concurrent Disorders Program (CDP) is a newly developed inpatient service that will provide care to persons aged 16 and older presenting with serious psychiatric disorders and active substance abuse disorder/dependency. An integrated care model ensures a seamless integration of psychiatric and substance abuse interventions.

  • POSITION SUMMARY:

    This full-time position would be a great leadership opportunity for a self-directed, highly motivated and results-oriented clinical leader who is innovative and creative. In alignment with our mission and values, we are looking for a Nurse Practitioner who excels in engaging people, promoting quality and safety, building successful teams, program development and evaluation. Reporting to the Manager, Inpatient Services, you will be responsible for the clinical leadership and coordination of the CDP, with accountability for day-to-day operations, patient access and flow, professional development and training, risk management and safety, the provision of high-quality care and, ultimately, delivery of an exceptional patient experience.

    KEY RESPONSIBILITIES:

    Leader

    Collaboratively with the Medical Lead, develops, implements and evaluates annual operating plans, monitoring progression towards short- and long-range objectives, programming schedule (individual and group) and service enhancements, monitors effectiveness based on target outcomes.

    Collaboratively with the Medical Lead, develops, implements and evaluates policies, procedures, protocols, and business processes, e.g., referral management, assessment tools, withdrawal management, therapeutic programming, individualized patient service plans, etc.

    Leads the development, implementation and evaluation of patient safety, quality improvement initiatives and establishing key performance indicators and program scorecard.

    Works closely with community partners, referral sources and aftercare providers, develops and maintains effective linkages and coordinates services with community and regional partners to ensure a seamless continuum of care.

     Promotes a high quality care and work environment that encourages open and transparent interprofessional collaboration.

    Educator

    Identifies entry-level and advanced clinical competencies required by core nursing and allied health team members and broader clinical team.

    Develops and implements a program professional development and training plan, incorporating internal and external resources, which provide the greatest value-for-money.

    Evaluates the effectiveness of program professional development and training plan with closing the competencies gap, adjusting teaching/learning strategies as required.

    Provider

    Providing admission and on-going assessments, diagnosis, treatment, including prescribing medications and diagnostic tests and evaluation of primary care services for patients, including working collaboratively with other primary care providers, such as physicians and psychiatrists, communicating diagnoses and working with the interprofessional team in the development of a patient specific plan of care.

    Prescribes pharmacotherapy, within the NP scope of practice, based on the patient’s/patient’s health history, health problem/disease, disorder, condition or life stage, and individual circumstances.

    Provides patient diagnostic information and education that are relevant and evidence-informed, using appropriate teaching/learning strategies.

    QUALIFICATIONS

    Registered Nurse Extended Class (Nurse Practitioner) in good standing with the College of Nurses of Ontario (CNO). Extended class specialty certificate in Primary Health Care. No restrictions on registration or professional practice hearings pending.

    Mandatory Liability Insurance that meets CNO requirements for NPs ($5M coverage per setting).

    Successful completion of a CNO-approved course to legally enable prescribing of controlled substances.

    Five plus years of demonstrated experience as a Nurse Practitioner is required.

    Five plus years of relevant addictions and mental health clinical experience.

    Demonstrated values-based leader, committed to a results-oriented and patient-focused culture.

    Demonstrated knowledge of relevant legislation and commitment to the principles of the CNO Code of Conduct.

    Solid knowledge of quality, risk and improvement methodologies, e.g., LEAN, IRM best practices, process mapping.

    Demonstrated advanced training/experience providing evidence-based therapies for concurrent disorders.

    Excellent critical thinking, problem-solving, organizational and time management skills.

    Solid coaching, mentoring and clinical supervision skills to support new hires, early career practitioners and learners.

    Strong team and network building skills, create effective working relationships and partnerships.

     Proficiency in the Microsoft Office suite of applications.

     Must be able to meet the physical demands of the position.

     Valid driver?s license and access to a vehicle.

    Double vaccinated with a Canadian recognized COVID vaccine.

     Clear Police Records/Vulnerable Sector Check (must be completed within last 6 months).
