QCH is a delivery partner for the Ontario Health East region’s rollout of electronic referral (eReferral), in alignment with Ministry of Health (MOH) strategy to support front line providers making more efficient referrals and facilitating smoother transitions between health care settings for patients. This initiative is underway in collaboration with the Ontario eServices Program Office, which provides provincial oversight and coordination.
The Project Coordinator will be a member of the regional team under the direction of the Champlain eReferral Lead, and play a key role in supporting the successful implementation and adoption of eReferral technology. The Project Coordinator will provide administrative support for the team, including but not limited to engagements/meetings, action registers, and organization of key information.
- Provide point of contact for information and assistance to the Regional Team’s internal/external customers – including redirecting phone/email inquiries to appropriate personnel
- Provide administrative support to the eReferral Deployment team
- Schedule meetings, take minutes, and track actions for the team
- Updating and maintaining project tools and templates
- Assists with development, execution, and adjustment of project work plans and schedules
- Assists with project issue and project risk tracking and management
- Ensures standards of practice are applied, including project documentation development and management of project artifacts
- Assists with regular reporting on projects for stakeholders
- Assists stakeholders to gather and document requirements
- Administers collaboration sites/tools for the team
- Monitors team workplan and actions and initiate reminders/follow-ups as needed
- Post-secondary education, or five (5) years of related administrative and/or project related commensurate experience in lieu of formal education
- At least three (3) years’ experience in a project coordination / project administration or project office role
- Working knowledge of project management methodology, practices, and procedures
- Experience with process improvement and workflow analysis
- Experience with managing collaboration tools (Microsoft Sharepoint experience would be an asset)
- Strong organizational and time management skills
- Ability to effectively communicate both verbally and in writing in English
- Detail oriented with the ability to multitask through planning/organizing
- Effective facilitation and reporting skills
- Good analytical and problem solving skills
- Well versed in use of virtual meeting tools (eg, Zoom, Teams), computers and basic troubleshooting in the work environment
- Strong command of MS Office software

- Bilingual in English and French
