SOCIAL WORKER
McCall Centre for Continuing Care
Candidates are invited to apply for the position of Social Worker (full time) for McCall Centre for Continuing Care, a 141 bed facility (comprised of 120 Complex Continuing Care beds and 21 Interim Long Term Care beds), in Etobicoke, Ontario. McCall Centre is managed by Extendicare Assist, on behalf of Trillium Health Partners Hospital.
Reporting to the Administrator, the Social Worker provides social work services for residents and patients, and families within legislative and budgetary parameters. The Social Worker also works in collaboration with hospital admissions coordinators and discharge planners to ensure patient flow in the complex continuing care beds, based on hospital capacity.
RESPONSIBILITIES
- Provides social work services to residents and patients family along the continuum of care from admission to discharge. Acts in an advocacy role as appropriate.
- Provides individual counseling and group sessions for residents and families as identified – i.e., orientation adjustment programs, crisis problem solving, conflict resolution / mediation, bereavement counseling, palliative care etc.
- In collaboration with the Director of Care, facilitates tours of facility
- In collaboration with the Operations/Office Manager, assists with admissions and adjustment issues.
- Acts, as a resource person to staff who have identified needs for counseling, referrals or support.
- Participates in providing in-service programs for facility staff, residents, and patients, and families as needed.
- Liaises with educational institutions and provides supervision to social work students on clinical placement.
- Participates on committees and teams related to resident care and operation of the facility as required (i.e. management team, customer service, palliative care, ethics etc.). Reports monthly on departmental activities.
- Liaises and consults with area hospitals, community social service organizations and government programs in relation to resident care issues and departmental activities.
QUALIFICATIONS
- Master’s (preferred) or Bachelor’s degree in Social Work from an accredited University;
- Previous experience in a long-term care setting an asset
- Previous management experience an asset
- Knowledge of applicable legislation, individual counseling and group sessions.
- Knowledge of occupational health and safety practices, principles and legislation.
- Computer literate
- An effective communicator, problem-solver, leader and diplomat.
